NEW STUDENTS/TRANSFEREES

STEP 1. Accomplish the registration form

STEP 2. Pay the Testing Fee. Take the Assessment Test.

STEP 3. After passing the assessment test, submit the accomplished Registration form with the following documents:

1. Original PSA Birth Certificate

2. SF/Form 138 (Report Card) with L.R.N.

3. ECCD checklist with L.R.N for incoming Grade 1

4. Certificate of Good Moral

5. Certificate of no Financial Obligation with school seal (if from private school)

6. Medical Records (for those under mainstreaming program)

7. Alien Certificate of Registration (ACR) for foreign students only

8. 2x2 picture (2 pcs)

STEP 4. Interview then accomplish the Enrollment Form

STEP 5. Proceed to the Cashier for payment (Tuition fee and books)

STEP 6. Get your school and P.E. Uniform.
